What is it
Usually the discomforts related to mammary hyperplasia occur at puberty, in some cases with the first pregnancy, even the sudden and significant weight gain due to excess fatty deposits, or in case of excessive weight loss.
Reduction mammaplasty is the only surgery that can solve the problem of breast hyperplasia and gigantomastia, in a definitive way.
Reduction mammaplasty is an aesthetic breast surgery procedure aimed at reducing the volume of the breasts and restoring harmony and tone to the breasts. During the operation the surgeon will proceed to remove excess fat, skin and glandular tissue in order to obtain a proportionate breast and alleviate the discomfort caused by breasts too large (breast hypertrophy).
In summary, reduction mammaplasty is recommended in the case of:
- Overdeveloped breast (Gigamtomastia)
- Excessive breast volume (mammary hypertrophy or hyperplasia) out of proportion to the rest of the body
- Pendulous and heavy breast, with areolas pointing downward
- Back pain due to the weight of the breast and displaced center of gravity
- Neck and shoulder discomfort caused by the weight of the breasts

There are three types of breast hypertrophies:
- Pure mammary hypertrophy: increased mammary gland, frequent in adolescents;
- Mixed hypertrophies: glandular component with excessive adipose tissue, frequent in postpartum women
- Adipose mammary hypertrophies: due to overweight or obesity.
Treatment
Breast reduction surgery consists of the proportionate removal of excess breast tissue, skin, fat and glands. The average duration of breast reduction surgery is about 1 hour, but it can also last for several hours. The reductive mastoplasty practiced with local anesthesia, is normally expected a day-hospital, if necessary the intervention under general anesthesia, is expected to stay in the clinic for one night.
Before resorting to breast reduction surgery, it is customary to undergo a specialist medical examination, in which the surgeon evaluates:
- The shape and volume of the breasts
- Skin type
- The quality of breast tissue and the position of areolas and nipples.
Next, the medical history is taken, the surgeon explains the surgical technique used, and prescribes specific clinical examinations (electrocardiogram, breast ultrasound, mammography, and others) in order to carefully assess blood pressure and healing capacity.
The physician recommends nutrition and prescribes drug therapy to be followed in the pre-operative and post-operative phases. Non-steroidal anti-inflammatory drugs (NSAIDs), aspirin, should be avoided, as they can interfere with the metabolism of anesthetics, and the surgeon recommends to stop smoking for a certain period before and after surgery, to facilitate a faster recovery.
The surgery does not alter the integrity of the mammary gland, so there are no contraindications for breastfeeding, except in the case of excessive reduction.
If the patient's state of health is not compromised, the patient gives her consent to breast reduction surgery.
The goal is to remove excess breast tissue until a proportionate breast of the right size is achieved.
After the operation there may be a slight swelling and bruising, which disappear in the first 2 weeks.
In case of pain in the breast and in the movements of the arms, it is possible to take analgesics appropriately prescribed by the surgeon after surgery;
The resumption of activities, depending on your individuality is between 7 and 15 days, avoid lifting heavy loads and perform strenuous tasks.
Procedure
Before proceeding to the reduction mammaplasty operation, the anesthesiologist will examine the patient in order to review the clinical examination and confirm the execution of the operation.
Before the local anesthesia with intravenous sedation, the surgeon draws on the patient's breast, the lines on which will be drawn the incisions during surgery. (The incisions are made only around the areolae and then vertically downwards to the sub-mammary fold and along the lower breast fold.
The surgeon makes a single incision around the nipple and a vertical one under the nipple, at which point he removes any excess fat, part of the mammary gland and excess skin.
After the removal, the surgeon reshapes and repositions the nipple in the area predetermined in the pre-operative phase. Usually this also involves a resizing of the areola, so that it appears proportionate to the new size of the breasts.
At the end of the operation, sutures are applied around the nipple and on all areas of the incision (the lines drawn vertically to the areola and along the sub-mammary groove).
We proceed to the application of drains, which will be maintained for about 1 or 2 days, whose function is to eliminate the blood accumulated during the operation. After a few days the patient will wear a specific bra.
The ability to heal varies depending on the individual characteristics of the patient.
The night after breast reduction surgery, the patient will still be under sedation and will therefore need to be assisted, moderate food intake is recommended. Stiffness of the operated area, fatigue and slight swelling are typical discomforts of the post-operative phase, which are considerably alleviated by the anti-inflammatory drugs prescribed by the doctor.
A first dressing with the surgeon is scheduled within the first week after surgery and a second dressing for the removal of stitches, the following week. It is recommended to wear the compression bra for reduction mammaplasty for 1 month, to support the breast in the healing process and minimize swelling.
